Policies

Hours

Hours are by appointment.

Length of Session

One-hour sessions last for one full hour (60 minutes).

Half-hour sessions last for a full 1/2 hour (30 minutes).

15 minute sessions last for 15 minutes.
(Initial visit must be at least 30 minutes)

If time is available, sessions can be extended at the client's request.
Sessions cannot be shortened (without 24 hours notice).  Client is responsible for time reserved.

 

 

Fees

 Relaxation (Swedish) Massage 
(also includes pregnancy massage and deep tissue massage)

$40 for a half-hour session (full thirty minutes)
$60 for a one-hour session (full sixty minutes)
$25 for each additional half-hour

Specialized Therapy
(treatment of a specific problem / any technique other than relaxation massage)

$20 for a 15 minute session
$40 for a half-hour session
$80 for a one-hour session
$20 for each additional 15 minutes

for Home Visits

$120 for a one-hour home visit with 30 minutes or less travel involved.

Discounts

Discounts are available for senior citizens (age 65+).

Discounts are for office visits only.  Prices/Discounts are reviewed each year.

Important Notice:  
To avoid Ohio state sales tax, please bring a written order from your M.D. or Chiropractor for massage.  If you call your doctor and ask for one, He/she should be happy to comply.  It should NOT have a set number of visits.  It can say something as simple as "Recommend massage for stress reduction".  Without a doctor's order, massage is subject to 6.5% sales tax, which will be in addition to the above prices.

Payment options

Cash, Check, Visa, MasterCard and Discover are accepted. 

Payment is due at the end of the session.

(Gift certificates may be purchased in person or by phone)

Returned Checks

There will be a $20 charge for returned checks

Missed Appointments

24 hours notice is required for cancellation.  If a time slot is saved for you, other clients were probably turned away in order to hold a space for you.  If you miss your appointment, or cancel on the same day, You will be charged $40 for the missed appointment.  If the time slot is filled at the last minute, you will not be charged.

Late for appointment

Since only a certain amount of time is reserved for you, your late arrival means that your appointment will be shorter.  The cost will remain the same.

If the therapist is late

If the appointment is missed, or begins late, because of the actions of your therapist, the session will be extended or the price adjusted.  This would be a rare occurrence.

Inappropriate behavior

Rude, or sexually suggestive behavior will not be tolerated.  Inappropriate behavior will result in the session being terminated.  If this occurs, you will be expected to pay for the session.

Privacy

Records

Your medical history form and records are kept in a file cabinet that is only accessed by your therapist. 

Computer Records

Computer records do not include medical information. 
A password is required to access personal contact information. 

The computer is also protected by both a firewall and an anti-virus software program.  I do not engage in electronic billing.

Sharing information

Your personal information is shared with insurance and health maintenance organizations or with other medical professionals only when necessary and only with your written authorization.  Your records can be subpoenaed by a court and so are not strictly confidential.
